Save time and help prepare for your visit by completing the 4 steps below* in advance of your appointment*. If you do not have access to a printer/computer: please plan for extra time to complete the paper and online forms with our in-office iPad before your appointment, and please bring your spine-related medical records and imaging on a disc/CD.
